The Accident Report Form is to inform FLM of any accidents/injuries that occur on the FLM campus.
- The Accident Report Form must be filled out in its entirety to be valid. This form should only be used to report any accidents or injuries on the FLM campus. Failure to fill out this form properly will hinder the process of receiving the attention of the FLM office. >>
